| Members shall have toll free access to the EA-USA multilingual coordination center 24 hours a day, 365 days a year |
| Upon a Member's request, EA-USA shall provide the names, addresses and telephone numbers of physicians, hospitals, dentists, and dental clinics in the area the Member is traveling. EA-USA shall also attempt to confirm the availability of the provider, ascertain required payments which a Member shall be required to pay and make an appointment for a Member with the medical provider of the Member's choice. |
| EA-USA shall arrange but not pay for the Member to receive his or her eyeglasses that the Member may have forgotten or for any other reason does not have and needs for the enjoyment of his or her trip. All costs of the eyeglasses, including shipping charges shall be home by the Member. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member |
| If a Member has an unexpected need for prescription medication while on a Covered Trip, or loses, forget or runs out of prescription medication while traveling, EA-USA shall attempt to locate the medication or its equivalent and attempt to arrange but not pay for the Member to obtain it locally, where it is available or to have it shipped to the Member at the Subscriber's or Member's expense, subject to local laws, if it is not available locally. The Member shall be provided with a cost estimate for the replacement medication and/ or shipment costs that are subject to his/her approval. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member of the Subscriber |
| When notified of a Member's medical emergency resulting from an accident or sickness, EA-USA multilingual staff shall, whenever appropriate in the judgment of EA-USA or a physician designated by EA-USA, attempt to contact the Member and Member's local attending medical personnel in order to attempt to obtain a full understanding of the Member's situation and to attempt to monitor the Member's condition. EA-USA shall continue to monitor the Member's condition and shall remain in communication with the Member's family until the Member's medical emergency is resolved |
| The cost of the medical Services shall be borne entirely by the Members. However, EA-USA shall advance on-site emergency inpatient medical payments to the Member, up to U.S $10,000, upon receipt of a letter or guarantee of payment from the Subscriber. EA-USA is not responsible for the adjudication of medical bills incurred by the Member. Upon receipt, EA-USA shall pay said bills and re-invoice them to the Subscriber, who further agrees to reimburse EA-USA within 30 days of receipt of the invoice. If the Subscriber does not guarantee the expenses EA-USA shall advance payments upon receipt of a Satisfactory Guarantee of Reimbursement from the Member such as a credit card |
| EA-USA shall arrange and pay for the medical evacuation of a Member in the event of a medical emergency. Said evacuation, which shall be performed under medical supervision, shall be to the nearest medical facility capable of providing adequate care and shall only be performed if such care is not available locally. The determination of whether a medical condition constitutes an emergency and whether area facilities are capable of providing adequate medical care shall be made by physicians designated by EA-USA after consultation with the attending physician on medical factors and their decisions shall be conclusive in determining the need for such Services and the destination to which the Member is to be transported. The transportation arrangements for the medical evacuation shall be by the most direct and economical route possible and shall be arranged by EA-USA. EA-USA shall not reimburse the Member and/or the Subscriber for transportation and airfare arranged for and/or paid for by said Member and/or Subscriber. As part of a medical evacuation or repatriation, EA-USA shall also make all necessary arrangements for ground transportation to and from the hospital, as well as pre-admission arrangements, where possible, at the receiving hospital. EA-USA shall pay for the expenses pertaining to this Service up to the limit indicated in the insurance policy or program of the Member involved. All third party expenses and reimbursable will be re-invoiced to Subscriber who further agrees to reimburse EA-USA within 30 days of receipt of the EA-USA invoice (the “Due Date”). |
| If a Member dies, EA-USA shall arrange and pay for all necessary government authorization, including a container suitable for transport, and for the transportation of the remains, to the Member’s domicile. EA-USA shall determine the means of such transport. EA-USA shall pay for the expenses pertaining to this Service up to the limit indicated in the insurance policy or program of the Member involved. All third party expenses and reimbursable will be re-invoiced to Subscriber who further agrees to reimburse EA-USA within 30 days of receipt of the EA-USA invoice (the “Due Date”). |
| If a Member is hospitalized and if recommended by the attending physician, EA-USA shall arrange the transportation of one (1) immediate family member designated by the Member from his/her home to the place where the Member is hospitalized. If this Service is included in the Member’s policy, EA-USA will pay for the applicable third-party expenses up to the limit indicated in the insurance policy or program of the Member involved. Payable expenses include economy class common carrier transportation, necessary ground transportation, food and hotel. All third party expenses and reimbursable will be re-invoiced to Subscriber who further agrees to reimburse EA-USA within 30 days of receipt of the EA-USA invoice (the “Due Date”). |
| If a Member is traveling with dependent children, and such dependent children would be left unattended because of the hospitalization of the Member, EA-USA shall arrange and pay for the return of such dependent children to the Member's domicile. If necessary, EA-USA shall also arrange but not pay for the transportation and costs for a qualified attendant to accompany the dependent children. EA-USA shall pay for the expenses pertaining to this Service up to the limit indicated in the insurance policy or program of the Member involved. All third party expenses and reimbursable will be re-invoiced to Subscriber who further agrees to reimburse EA-USA within 30 days of receipt of the EA-USA invoice (the “Due Date”). |
| If a Member's “Traveling Companion” has lost previously made travel arrangements due to a delay caused by the Member's hospitalization, EA-USA shall arrange but not pay for the return of such Traveling Companion to his/her primary place of residence. A “Traveling Companion” is defined as a person with whom the Member is booked to share the same itinerary. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member. |
| If a Member is physically unable to operate any non-commercial vehicle (i.e., auto, motorhome, rental car, etc) as a result of a medical emergency, EA-USA shall arrange but not pay for the return of the vehicle left unattended to the Member’s domicile or place of rental. The vehicle must be in good driving condition and capable of being driven on the highway in compliance with local laws. The Member will be responsible for all expenses incurred by the proposed driver of the vehicle (limited to one person) to reach the vehicle including; transportation expenses for the driver to return to his/her domicile after the vehicle has been returned to the Member’s domicile or place of rental; fuel, tolls and parking expenses from the point of departure to the point the vehicle reaches the proposed destination; food and accommodation expenses for the driver from the point of departure to the point the vehicle reaches the proposed destination (alcoholic beverages are not included); or driving fees for a commercially-licensed driving agency. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member. |
| If the Member's pet, traveling with him/her, is left unattended as a result of the Member's hospitalization, EA-USA shall arrange but not pay to transport the Member's pet to his/her domicile. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member. |
| If a Member’s “Immediate Family Member” is critically injured, sick or dies while the Member is traveling, EA-USA shall arrange but not pay for the return of the Member to the Immediate Family Member’s place of hospitalization or burial via the most direct route on economy class airfare. The determination of the “Immediate Family Members” critical condition shall be made in good faith by EA-USA after obtaining permission from the Member to contact the attending physician caring for the “Immediate Family Member”. An “Immediate Family Member” means any children, adopted children, stepchildren, children-in-law, parents, stepparents, parents-in law, siblings, siblings-in-law, grandparents, grandchildren and legal spouse of the Member. Member or Member’s family shall be responsible for all expenses, including airfare. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member. |

| In the event of an emergency, EA-USA shall help the Members make emergency travel arrangements, including airline, hotel, and car rental reservations. The Subscriber or Member is responsible for payment of all tickets, accommodations and rentals arranged. Before the provision of this service, EA-USA shall obtain a Satisfactory Guarantee of Reimbursement from the Member. |
| Providing names and contact info of lawyers in the area, notifying the proper embassy or consulate of the members incarceration, continued communication with the incarcerated member, advancing bail funds upon the receipt of a guarantee of repayment by the member, arranging for the receipt of funds from third party sources |
| EA shall assist a member with the following items, however any third party expenses including shipping and/ or express delivery charges shall be the responsibility of the member; Locating lost luggage, obtaining a lost transportation ticket application, locating lost documents and other important personal items |
| If a member needs help communicating in an emergency EA shall provide translation services in all major languages via telephone to a Member. If a Member requires ongoing or more complex translation Services in emergency situations, EA-USA shall refer the Member to local translators. The cost of the local translators shall be borne by the Member. |
| In an emergency, EA-USA will use its best efforts to transmit an urgent message for a Member to the Member’s family, friends, and/or business associates. EA-USA will also accept and retain messages for Members at the EA-USA Coordination Center for up to fifteen (15) days. |
| EA-USA shall advance emergency funds, up to $500, to a Member upon receipt of Satisfactory Guarantee of Reimbursement from the Member. Any fees associated with transfer or delivery of funds are the responsibility of the Member |
